Integrating WMTY into a technology enabled Community Driven Approach to Health Navigation
Join colleagues from Montefiore Einstein and the Food Bank for New York City as they share their innovative partnership using a co-designed AI Chatbot and a WMTY approach to train Community Health Navigators to support meaningful, person-centered conversations. Presenters will describe how community-based navigators were equipped to uncover what matters most to community members, build trust, and connect individuals to health and social resources through actionable, strengths-based care plans.

Kody Gerkin, Point Sur Consulting representing the Food Bank of NYC
